Rochester, Mich. – Jan. 28, 2015 – Ewa Matuszewski, CEO of MedNetOne Health Solutions (MNOHS), a Michigan-based healthcare management organization serving the infrastructure and clinical support needs of private practice physicians and behavioral health specialists, will serve as a panelist at Inforum’s “What will health care delivery look like in the future?” event on Feb. 10. The event will examine the future of the health care industry and how health care organizations should embrace scientific, technological, consumer and social trends to change the way health care is delivered.

Matuszewski, a frequent speaker on innovative primary care and chronic care initiatives who regularly consults with medical societies and local, state, national and international health care organizations, will speak from the provider perspective on the panel.

MedNetOne Health Solutions announces clinical director and project manager for new High Intensity Care Management model

Rochester, Mich. – Nov. 12, 2014 – MedNetOne Health Solutions (MNOHS), a Michigan-based healthcare management organization serving the infrastructure and clinical support needs of private practice physicians and behavioral health specialists, announces Maureen Murphy, FNP – BC, has joined the organization as Director of Field Care Management Clinicians for the High Intensity Care Management (HICM) model and Lauren Yaroch, RN, has joined the organization as the HICM Project Manager.

MedNetOne Health Solutions is the leading participant in HICM, which was launched Oct. 1 by Blue Cross Blue Shield of Michigan (BCBSM). Started by the Centers for Medicare & Medicaid Services (CMS) at the national level, the HICM model is designed to offer team based health care, with a primary care physician in the lead, to select Medicare Advantage patients in the region who have at least six of 14 chronic condition diagnoses (co-morbidities).

In her new role as Director of Field Care Management Clinicians, Murphy oversees the implementation and ongoing activities of the HICM model in more than 80 MNOHS member physician practices.

Prior to joining MNOHS, Murphy worked for Henry Ford Health System as a clinical supervisor for the Children’s Health Project of Detroit Mobile Medical Unit. Before Henry Ford, she was employed with St. Johns Hospital as a floor nurse and specialized in the area of adult/elderly chronic health conditions. Other clinical experience includes serving as district school nurse/health care coordinator for Detroit Edison Public School Academy and as a labor and delivery nurse at Hutzel Hospital.

Murphy holds a Master of Science degree in nursing (Family Nurse Practitioner) and Bachelor of Science degree in nursing, both from the University of Detroit Mercy. In 2013, she received the Loretta Ford Nurse Practitioner Award from the University of Detroit Mercy for exemplifying the mission of the McAuley School of Nursing through excellence in academics, leadership in clinical practice and community service. Murphy has written and received numerous grants from the Michigan Department of Community Health (MDCH). She is a member of the Sigma Theta Tau International Honor Society of Nursing and the Michigan Council of Nurse Practitioners and is a frequent public speaker.

“Oversight of the High Intensity Care Management model requires high level clinical, communication and coordination skills. Maureen Murphy possesses not only these skills but the experience needed to ensure HICM goes smoothly as it is integrated as a sustainable model into multiple primary care practices,” Ewa Matuszewski, CEO of MedNetOne Health Solutions said.

In her role as HICM Project Manager, Lauren Yaroch coordinates clinical activities for HICM and participating MNOHS member physician practices. Prior to joining MNOHS, Yaroch worked for Botsford Hospital Center as a Registered Nurse in the Trauma Center/Emergency Department and as a nurse technician in the medical-surgical unit. She received her nursing degree in Applied Science from Henry Ford College and also completed the nursing assistant program at the Cadillac Technical Center in Cadillac, Mich. She holds certifications in basic life support and advanced cardiac life support.

MedNetOne Health Solutions participating in new High Intensity Care Management program with Blue Cross Blue Shield of Michigan

Rochester, Mich. –Nov. 4, 2014 – MedNetOne Health Solutions (MNOHS), a Michigan-based healthcare management organization serving the infrastructure and clinical support needs of private practice physicians and behavioral health specialists, announces it is participating in the new High Intensity Care Management model (HCIM) launched Oct. 1 by Blue Cross Blue Shield of Michigan (BCBSM). Started at the national level by the Centers for Medicare & Medicaid Services, HICM refers to a suite of services offered to the Medicare Advantage population and supported in Michigan by BCBSM and Blue Care Network. Greater Macomb Physician Hospital Organization is working in partnership with MNOHS on the HICM model.

Previously the domain of national disease management organizations, the new HICM model is designed to offer team based health care, with a primary care physician in the lead, to select Medicare Advantage patients in southeast Michigan who have at least six of 14 chronic condition diagnoses (co-morbidities). HICM identifies and serves patients through three levels of care:

1. Tier One: for patients who are stable and require a maximum of one primary care visit and/or one phone call per month
2. Tier Two: for patients who are moderately ill and require more regular follow-up and touch points with their primary care physician/team
3. Tier Three: for patients who are seriously ill and require weekly or daily contact with a primary care team member and/or a member of the extended team, such as an Advance Practice Nurse (AP-N), R.N., MSW, pharmacist or home care agency.

MNOHS CEO, Ewa Matuszewski, acknowledged the challenging scope of the HICM initiative.

“The High Intensity Care Management model is definitely ambitious, but is a logical step in the patient-centered, population-based care movement,” said Matuszewski. “The Medicare Advantage patients involved in the program have multiple physical and psycho/social health needs and will greatly benefit by a local, broad-based care team that can serve them on the health care continuum.”

Prior to the October 1 rollout, a select number of MNOHS physician group membersparticipated in a month-long preparatory phase of HICM. These practices worked with approximately 70 Medicare Advantage patients to determine how the initial HICM process outline would achieve the desired outcomes.

“The preparatory phase offered important insights on the adequacy of efforts to identify and capture all of the diagnoses for the Medicare Advantage patients assigned to the practice, to test run whether the communication strategy among the health care team can help achieve patient care goals, and how best to handle the transfer of patients into the HICM program,” said Matuszewski. “Since that time, the MNOHS team has seen nearly 900 patients through HICM, given the demand for comprehensive health care services for this population.”

MedNetOne Health Solutions hires director of behavioral health integration

Rochester, Mich.—Oct. 11, 2013—MedNetOne Health Solutions (MNOHS), a Michigan-based health care management organization serving the infrastructure and clinical support needs of primary and specialty care private practice physicians and behavioral health specialists, announces Lori Zeman, Ph.D., has joined the organization as director of behavioral health integration. Dr. Zeman is also a Clinical Assistant Professor at the Wayne State University School of Medicine, Department of Psychiatry and Behavioral Neurosciences.

In this new role, Dr. Zeman oversees the integration of mental health and substance abuse assessments and interventions into MedNetOne Health Solutions’ physician member practices and other medical settings to increase access to behavioral health services and address behaviors that can enhance or impede overall health and well-being.

“With Dr. Zeman facilitating behavioral health within our member practices, MedNetOne Health Solutions enhances the services we offer to physicians and their patients through a fully integrated, patient-centered medical home,” said Al Juocys, D.O., Chief Medical Officer of MedNetOne Health Solutions.

Dr. Zeman is active in the mental health community and serves on a workgroup focused on mental and physical integrated services delivery as part of Michigan’s Mental Health & Wellness Commission, created by Gov. Rick Snyder in February 2013 to strengthen mental health resources in the state. She obtained her Ph.D. in clinical psychology from the University of Miami and completed her pre-doctoral internship in pediatric health psychology at the University of Minnesota; she holds a Bachelor of Science degree from Gustavus Adolphus College in Minnesota.

MedNetOne Health Solutions hires innovation coordinator/ population coach

Rochester, Mich.—July 8, 2013— MedNetOne Health Solutions (MNOHS), a Michigan-based health care management organization serving the infrastructure and clinical support needs of primary and specialty care private practice physicians and behavioral health specialists, announces Mark Lazar has joined the company as Innovation Coordinator and Population Coach.

In this role, Lazar will be working with MedNetOne Health Solutions’ member practices to implement special projects including, population health, which is an approach to health care aimed at improving the health of an entire designated population, for example, all patients within a physician practice. Population health moves beyond individual-level focused care and complements the organized efforts of public health by addressing a broader range of factors that impact health on a population-level.

Lazar will work with physicians, behavioral health specialists, care managers and other health practice team members to gain a better understanding of the overall health, and factors affecting the health of, patients within MedNetOne Health Solutions’ practices. Using data as a key tool, he will advise on and implement the appropriate programs, resources and techniques required by MedNetOne Health Solutions member practices.

Immediately prior to joining MedNetOne Health Solutions, Lazar was a field service supervisor for Ohio CAT, the exclusive, authorized dealer for Caterpillar equipment and engines in Ohio and parts of Kentucky and Indiana. At CAT, he was a participant in the Caterpillar Flagship program, which prepared future Caterpillar leaders for executive-level positions. Prior to CAT, Lazar was a national sales representative for MARS music and a community support provider at Unison Behavioral Health Group.

Lazar graduated from the University of Toledo with a Bachelors of Arts degree in Psychology and a minor in sociology. He is also an Olympic development athlete, an Olympic trials competitor (2006) and a nationally ranked curling athlete (1998-current).
